Monroe CBOC Locum Tenens
The Department of Veterans Affairs, Overton Brooks intends to solicit proposals from Service Disabled Veteran Small Businesses (SDVOSB) to provide two (2) board certified or board eligible in Internal Medicine or Family Practice Locum Tenens physician for Primary Care, Overton Brooks VA Medical Center, Shreveport, Louisiana, Community Based Outpatient Clinic (CBOC) located in Monroe, Louisiana, in accordance with all terms, conditions, provision and the schedule of this solicitation, for the period from date of award up to 6 months, pending availability of funds. A VA security check is required. Required information for the VA security check is as follows: name of individual, address, date of birth & social security number. If the candidate has had a VA security check in the last five years, provide the evidence. Candidate must provide clinical references who can attest to candidate s clinical practice within the past year. BLS is required. Candidate must acquire Blue Cross Blue Shield (BCBS) provider identification number.
The VA anticipates issuing a solicitation for this requirement on or about July 03, 2017 with a closing date for proposals on or about July 24, 2017. The solicitation will be available for download from www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments.
The physician is to be competent in modern treatment principles and techniques, fully trained in all aspects routine adult medical care. The physician will work in Primary Care at the Overton Brooks VA Monroe Community Based Outpatient Clinic, 1691 Bienville, Monroe, LA 71203. The physician will see approximately 12- 15 patients per day in the clinic. The physician must provide 45-day notice of planned absence during the contract period.
The Government anticipates making a firm-fixed price contract for this requirement. North American Industry Classification System (NAICS) Code is 561320 and size standard is $27.5 Million and the solicitation will be issued for SDVOSB (set-a-side). All responsible sources may submit proposals which shall be considered by the Agency. To be eligible for award, contractors must be registered in SAM (System for Award Management) database (information on registration requirements can be obtained by the Internet at https://www.sam.gov and submit their complete Annual Representations and Certifications (ORCA) via online at https://orca.bpn.gov at proposal due date/time.
