This is not a Request for Proposal (RFP) or Request for Quotation (RFQ).
In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center located at 4212 N. 16th Street, Phoenix, AZ 85016 is conducting a market survey to identify contractors the following: Mobile Stereotactic Breast Biopsy Services. (Refer to attachment A for additional information.)
Interested contractors who meet the criteria above may submit information to the acquisitions department. This RFI is open from May 31, 2014 through June 20, 2014.
This RFI is for planning purposes only.
