This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation being requested. A written solicitation will not be issued. The solicitation number is W912JB-19-Q-0142. This solicitation is issued as a request for quotation (RFQ).

The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-04. This requirement is for full and open competition for other than small business. The associated NAICS code is 621399.


The prospective awardee must have an active registration in www.sam.gov at the time of award. Vendors are encouraged to verify that their registration is active prior to submitting a quote. If the prospective awardee is not active in SAM data base at the time of award, the prospective awardee will be allowed 5 calendar days to become registered and active in SAM. The award will proceed to the next otherwise successful registered and active offeror if the prospective offeror does not become registered and active by the 5 calendar day deadline.


This is a firm fixed price requirement. Refer to the Performance Work Statement for detailed information on the requirements.

Contractor within 50-mile radius of Lansing, MI shall provide job-related medical examinations who could potentially be exposed to health hazards in the work. 12-month period of performance.


0002    CONTRACT MANPOWER REPORTING    1    job    
The contractor shall report all contractor labor hours (including subcontractor labor hours) required for performance of services for the National Guard Bureau via a secure data collection site. The contractor shall completely fill in all required data fields using the following web address: http://www.ecmra.mil/. Reporting inputs shall be for the labor executed during the period of performance during each government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk at http://www.ecmra.mil.

One month period of performance                
CLIN 0001 and 0003 are the same service; CLIN 0003 for a one-month period of performance would be in effect if clause 52.217-8 would be exercised for one month or up to the max of six months.


Due to the majority of personnel being in the Lansing area, medical facility should be within a 50-mile radius of Lansing, Michigan. Current Wage Determinations are applicable.


Inspection and acceptance will be at Lansing, Michigan. Performance will begin 30 days after the award date. This award is a service contract set up for multiple invoices to be submitted. Each cost CLIN is identified as 1 JOB which provides for multiple invoices to be submitted at the time of performance. Invoices are to be submitted for services performed; invoices will not be accepted for billing future services during the period of performance. Invoices must be submitted through Wide Area Work Flow (https://wawf.eb.mil).


Quotes must include price, technical capability and past performance information (PPI). Award will be made to the quote that presents the best value to the government. Price will be evaluated, along with technical capability and PPI. The awardee must not have negative or adverse information on FAPIIS or SPRS in the last three years. Technical capability and past performance will be rated as Acceptable or Unacceptable. The Government intends to award to a large business that meets or exceeds the technical specifications listed and meets the provisions of the solicitation. The Government may award to other than the lowest price quote. The perceived benefits of the higher priced quote may merit the additional cost. Negative or adverse action in FAPIIS or SPRS in the last three years may render your quote unacceptable.


Award may be made without discussions. Therefore, vendor must provide complete information in the package for the Government to determine the best value. The Government reserves the right to require minor clarifications. However, the Government may award without requesting clarification for ambiguity.


QUOTATION PREPARATION INSTRUCTIONS:
Complete the Quote Template - Laboratory Tests by filling in the price for each test. Do not remove the formulas in column D. If additional pricing is to be included in the quote, insert additional rows and an extended value. Refer to the Solicitation Clauses for 52.212-1 Instructions to Offerors for further instructions.


(1)    Pricing
    Price quotes must be firm-fixed price.
    FOB destination.
    Prices must be valid for 60 days.
    Terms are Net 30 days.


(2)    Technical information/descriptive literature. Offerors must submit technical information (descriptive literature/specifications) that adequately demonstrates their quote meets the minimum standards required as specified in the Statement of Work. A general statement of compliance or restatement of the requirements is insufficient. Ensure the descriptive literature addresses each salient characteristic listed.


(3)    Past Performance. Each offeror's past performance will be reviewed to determine ability to perform the requirements. The quote will be evaluated on the offeror's business practices, customer relationship, and ability to successfully perform as proposed. In assessing performance, the evaluator may (1) evaluate present and past performance information through the use of questionnaires completed by the offeror's references; (2) use data independently obtained from other government or commercial sources such as Past Performance Information Retrieval System (PPIRS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other data bases; (3) interviews with Program/Project Managers, Contracting Officers or rely upon personal business experience with the offeror; and (4) consider any information the offeror submits which explains any problems encountered on the identified contract and the offeror's correction actions. The government reserves the rights to obtain and evaluate past performance information from any source it deems appropriate.


The Government may not consider quotes that fail to include all of the information requested in this solicitation.


Questions are due by 4:30 pm, Eastern Time, 10 September 2019, and must be submitted via email. Questions will not be answered via telephone. Offers are due at 12:00 pm, Eastern Time on 17 September 2019. Email quotes directly to bonnie.l.reineer.civ@mail.mil. Do not submit quotes on FBO; they will not be accepted. It is the vendor's responsibility to monitor this site for any amendments. The government is not responsible for any attachments that are rejected by the cyber security due to corruption or virus extensions. It is the offeror's responsibility to contact the POC to verify the quote is received PRIOR to the date and time set for the due date of RFQ.
