Medicaid Case Management Services
As part of CYFDs Home Visiting Services program, this Request for Proposal is to solicit for the provision of medically necessary Case Management services to Medicaid eligible pregnant women and their infants up to sixty (60) days following the end of the month of delivery. Additionally, services may be provided to Medicaid eligible children under the age of three (3) who are medically at risk due to family conditions and who are not developmentally delayed.
