(ii) The solicitation number is W91RUS-18-Q-0004. This solicitation is a Lowest Price Technically Acceptable (LPTA) Request for Quote (RFQ) IAW FAR Parts 12 and 13.5.
(iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-96.
(iv) This is an unrestricted acquisition. The applicable NAICS code is 517311 and small business size standard is less than 1500 employees.
(v) The contract line item numbers and items, quantities and unit of issue are identified within Section B of the attached SF1449 and Pricing Schedule. (vi) This Request for Quote is for local exchange services for the Dugway Proving Ground, UT Dial Central Office, to include a variety of local circuits and services, including Direct Inward Dialing (DID) numbers, Local Exchange Digital Trunks, and Commercial Subscriber Lines. Potential offerors must be certified by the State of Utah to provide local exchange services.
