Source Sought

This is a Source Sought notice. This notice does not constitute a request for proposal, request for quote or invitation for bid. This Source Sought is a market research tool being utilized to determine the availability of qualified Buy-Indian owed business concerns prior to issuing a Request for Quote. This is not a solicitation for proposals and no contract will be awarded for this announcement. No reimbursement will be made for any cost associated with providing information in response to this announcement. All responses will be provided by e-mail response. No presentation will be scheduled.


In accordance with the Federal Acquisition Regulation 5.2, the Great Plains Area Indian Health Service Diabetes Program is conducting a Source Sought notice to provide in person training and ongoing web-based learning Lifestyle Coach Training for the Diabetes Program. To provide a Master Trainer to facilitate between 1-3 Lifestyle Coach Training (s) in a meeting space. The Master Trainer will lead the two-day training session with a group of up to 18 participants.
This is service will take place in Aberdeen, SD dates and locations will be determined upon award.
The North American Industry Classification System (NAICS Code are 611-Educational Services, 611710 Educational Support Services, with a Small Business Size Standard of $15.0 million.
The written response to this notice should consist of following items.


A. Company Name
B. Company Data Universal Numbering System (DUNS) number
C. Company point of contact, mailing address, e-mail address, telephone number, fax number and website address
D. Name, telephone number, and e-mail address of a company point of contact that has the authority and knowledge of clarify response with government representative.
E. Type of company: All respondents should be registered on the System for Award Management (SAM) located at www.sam.gov.
Respondents will not be notifying individuals on the results of the source sought.

It should be clearly understood that this a Source Sought notice this is not an award or commitment by the Government. The offeror is further advised that funding may not become available. If funds are not available, no award will be made. The offeror will not be reimbursed for any or all effort or submission of preparation cost.
