Life Insurance Services
The City's Human Resources Department is soliciting proposals from qualified firms to provide Life Insurance Services to benefit eligible employees of the City. Services will be extended to the legal spouse or qualified domestic partner and all benefit eligible dependent children. The City has approximately 15,000 employees who would be eligible to participate in the life insurance program. This solicitation seeks a provider of services for the period of August 1, 2010, through July 31, 2013, with options to renew for two additional one-year periods. The City of Phoenix is seeking one vendor to provide all required services contained in this Request for Proposal (RFP).
