This Grant aims to develop the capacity of organizations to incorporate evidence-based strategies into their existing service array to build a community health system that will support the utilization of tobacco-cessation, cancer, cardiovascular disease/stroke, pulmonary disease, and diabetes services within targeted lesbian, gay, bi-sexual, and transgender (LGBT) communities.
