LOCUM NURSE PRACTITIONER SERVICES
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6-Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.5, Test Program for Certain Commercial Items; FAR Part 12, Acquisition of Commercial Items; and FAR 37.4 - Nonpersonal Health Care Services, in which the contractor is an independent contractor.
The Billings Area Indian Health Service (IHS) intends to award a Fixed Price Commercial Item contract in response to Request for Quotation (RFQ) 10-17-044-RDJ.
The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-94. This solicitation is restricted to 100% Small Business concerns. The associated North American Industry Classification System code is 561320 with a small business size standard of $27.5 million.
Quotations should be prepared in accordance with Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offerors - Commercial Items (JAN 2017).
PRICE SCHEDULE - Locum Nurse Practitioner for the Wind River Service Unit, PHS Indian Health Center, Fort Washakie, Wyoming. Offerors are requested to propose an all‑inclusive hourly rate when submitting a quote inclusive of transportation, meals, insurance, per diem and lodging.
PERIOD OF PERFORMANCE: October 1, 2017 to September 30, 2018, with four 12-month options. Option year dates of services will be determined upon contract award.
52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.
PURPOSE OF THE CONTRACT: The purpose of this acquisition is to contract for a Locum Tenens to provide a Nurse Practitioner for the Wind River Service Unit, PHS Indian Health Center, Fort Washakie, Wyoming. The work schedule is full-time and will be coordinated with the Clinical Director / Chief Medical Officer.
INTRODUCTION
The Wind River Service Unit provides ambulatory health care for the service population on the Wind River Indian Reservation. The Nurse Practitioner will provide direct and comprehensive primary, preventive and therapeutic medical and nursing care to individuals across the lifespan.
WORK SCHEDULE: The contractor shall provide services at the Fort Washakie Health Center, Fort Washakie, Wyoming. Any leave or vacation by the contractor shall involve consultation with the Government by the Contractor, but shall be at the convenience of the Government. Notification of leave request must be given at least two (2) weeks prior to the date of change.
