This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
This solicitation, W912JC-18-T-5703-LABORATORY SERVICES is being issued as a request for quotation (RFQ). Quotes/Pricing schedule must be good for entire Period of Performance.
(The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-60 and are found either in the Federal Acquisition Regulation (FAR); Defense Federal Acquisition Regulation Supplement (DFARS).
This procurement is 100% small business set-aside. The NAICS code is 621511, Laboratory Testing Services, Medical. The small business size standard is $32.5 Million.
The solicitation and award are subject to availability of funds.

All interested parties must submit pricing schedules for LABROTORY SERVICES. Types of test will be determined by physician during annual exam. See Statement of Work for breakdown and example of testing needed.
A single award Blanket Purchase Agreement will be made as a result of evaluation of quotes received. The Government will award a Firm Fixed Price (FFP) contract. Advance payments are not authorized.
The commercial items being requested shall be priced as FOB destination to 2737 S Kansas Ave, Topeka KS 66611.
The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items (JAN 2017), applies to this acquisition. All companies must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. Vendor to provide a list of specifications for all items submitted.
The provision at FAR 52.212-2, Evaluation--Commercial Items (OCT 2014), applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
• Technical acceptability. "Equal" products must meet Government minimum specifications (See attachment #1 SOW)
• Price
The provision at FAR 52.212-3 (b), Offeror Representations and Certification--Commercial Items (JAN 2017), applies to this acquisition. All vendors must have completed the annual representations and certifications electronically via the System for Award Management (SAM) Web site located at http://www.sam.gov/portal.
The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (FEB 2012), applies to the acquisition.
The clause at FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (JAN 2017) applies to this acquisition.

Responses to this solicitation must be submitted no later than 10:00 AM, CST on 29 Nov 2017.
All questions regarding this solicitation must be submitted by 8:00 AM, CST on 27 Nov 2017.
In the event adequate Small Business contractors are not available for adequate competition for this project, it will be re-advertised as unrestricted.
