This is a combined synopsis/solicitation for additional workstation services prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFP) RF-2019-0004. This solicitation is issued as a "BUY INDIAN SET-ASIDE."


(iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16.


(iv) The associated North American Industry Classification System code is 541490.


(v) PRICE/COST SCHEDULE - Kayenta Health Center - Additional Workstations.


(vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide services to the Kayenta Health Center, in Kayenta, AZ 86033.


(vii) Date and Place of Delivery: The Period of Performance will begin The Date of Award through 12/31/2019; Kayenta Health Center, U.S. Highway 160, South of Mile Post 394.3, Kayenta, AZ 86033.


(viii) PERFORMANCE WORK STATEMENT: SEE ATTACHED STATEMENT OF WORK, INCLUDES FLOOR PLANS.

(ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered: Technical Capability, Logistics, and Price.

3. Price: Technical factors are significantly more important than price. Price will be used as a form of a tie-breaker, should two or more responses be equal in points.


CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.


AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
