KUPUNA CARE CASE MANAGEMENT SERVICES
The Hawaii County Office of Aging is requesting proposals from qualified applicants to provide Case Management services under the State of Hawaiis Kupuna Care Program, to eligible persons sixty years or older living in the County of Hawaii. The contract term will be from July 1, 2009 through June 30, 2010 and may be extended for up to one (1) 12-month term. Multiple contracts may be awarded under this request for proposals.
