Combined Synopsis/Solicitation
Picture Archiving & Communications System (PACS)
i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 and Subpart 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; submissions are being requested and a written solicitation will not be issued.
ii. Solicitation no. FA5000-16-R-0012 is issued as a request for quote (RFQ).
iii. This solicitation document and its incorporated FAR provisions and clauses are those in effect through Federal Acquisition Circular 2005-84, effective 5 Oct 2015. All incorporated DFARS provisions and clauses are those in effect through DPN 20151001, effective 1 Oct 2015. All incorporated AFFARS provisions and clauses are those in effect through AFAC 2015-1001, effective 1 Oct 2015.
iv. Contract line item numbers, items, quantities, and units of measure may be found in Attachment 1, Combined Synopsis/Solicitation (COMBO). Offerors shall complete and return Attachment 1.
v. The Government anticipates a single award firm fixed price contract for PACS on Joint Base Elmendorf-Richardson (JBER). Please see Attachment 2, Performance Work Statement for detailed specifications of the Government's requirement and Attachment 3, SCA Wage Determination, for local wage determinations. Attachment 4, Workload Estimates, is for offeror reference only.
vi. All services shall be performed on Joint Base Elmendorf-Richardson (JBER), AK 99506 as specified in Attachment 2, Performance Work Statement.
vii. The offerors shall comply with all terms of the Request for Quote with particular emphasis on provisions FAR 52.212-1 & FAR 52.212-2 in Attachment 1.
viii. The offerors shall submit quote in accordance with the attachment no later than 16 February 2016, 4:30 PM Alaska Standard Time. Offers shall be submitted to kathryn.cortes@us.af.mil and nicholas.d.tapp@us.af.mil. If offers are mailed, the offer shall still comply with the submission time and date requirements.
