Inventory for Client and Agency Planning (ICAP) and Support Intensity Scale (SIS) Services
This Contract, by and between the State and Contractor is for the provision of transition from the Inventory for Client and Agency Planning (ICAP) assessment tool to the Support Intensity Scale (SIS) assessment tool, provision of assessments, and related services, as further defined in the SCOPE OF SERVICES.
