Integrated Care Program
The Illinois Department of Healthcare and Family Services (Department) is requesting proposals from responsible, qualified, experienced and financially sound Health Maintenance Organizations (HMOs)to enter into risk-based contracts to provide the full spectrum of Medicaid covered services through an integrated care delivery system to Older Adults and Adults (aged 19 and over) with Disabilities who are eligible for Medicaid but are not eligible for Medicare, in the counties of Lake, Kane, DuPage, Will, Kankakee and suburban (areas with zip codes that do not begin with 606) Cook county.
