Installation Ambulance Services
This is a request for information and capabilities only. This is NOT a request for proposals and no contract shall be awarded based on this sources sought notice. Army Contracting Command-Aberdeen Proving Ground (ACC-APG), Huachuca Division, Directorate of Contracting (DOC) Operations Branch is conducting market research in order to determine the availability, capability and interest of municipal entities/companies to conduct ambulance services on the Fort Huachuca Army Installation. No reimbursement will be made for any costs associated with providing information in response to this sources sought notice or any follow-up information requests. ACC-APG Huachuca Division's determination on how to best compete the proposed requirement will be based on responses to this notice and is the sole discretion of the Contracting Officer.
Work to be performed under this contemplated firm fixed-price contract will be within the North American Industry Classification System (NAICS) Code 621910 (Ambulance Services), with a small business size standard of $15,000,000.00. Responses to this notice must be received no later than 2:00 p.m. MST on January 20, 2017. Responses shall be e-mailed to Tie H. Steele at tie.h.steele.civ@mail.mil. No telephonic responses or personal visits please, due to multiple other contract actions in progress at this time.
Responses shall include the following on one page or less:
a. Sources Sought Notice Number,
b. Company/Municipal entity name and point of contact (POC), to include POC position title and POC e-mail address,
c. Arizona Department of Health Services (ADHS), Ground Ambulance Services, Certificate of Necessity (CON) Number,
d. DUNS or CAGE Number,
e. Number of years providing Ambulance Services.
