Conduct immunization site evaluations at a minimum of 100 private physician offices statewide who are active providers in the Vaccines for Children Program (VFC). Site evaluations will be conducted according to the Centers for Disease Control and Prevention’s (CDC) Assessment, Feedback, Incentives and eXchange (AFIX) guidelines.
