Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued.

This a sources sought/solicitation request for quotations (RFQ).

The associated North American Industrial Classification System (NAICS) code for this procurement is 339113, with a small business size standard of 750 employees.

The Asheville VAMC is seeking to purchase brand-name only Idylla Assay Kits.

EQUIPMENT REQUIRED:
Requirements

Vendor must be able to establish a shipping schedule (in coordination with Histology Supervisor and/or Lead technician) that allows for kits to be shipped in a timely manner.

The vendor must be approved by Idylla to provide assay kits for the Biocartis analyzer.

NETWORK / SECURITY REQUIREMENTS
All vendors are required to submit VA Form 6550 and Manufacturer Disclosure Statement for Medical Device Security (MDS2) Form with their solicitation if any component of proposed system requires connection to the VA network or retains patient information.

SERVICE
Vendor must obtain and show proof of National or Point to Point VPN certification. Any offsite server or network maintenance or support provided by the vendor can only be done via VPN access.

During the first year of service, on-site support service will be available 7 days/week with phone support available 24/7. If it is determined that a service call is necessitated, the vendor will have a service representative onsite within 24 hours. During the warranty period, the vendor will perform the required Preventative Maintenance on the instrument.

It is expected that an extended warranty will be activated upon implementation for a period of one year and during years 2-5, the vendor will provide upfront quotes for the annual costs of the service agreement/warranty.

During the warranty period (including the initial and option years), the vendor will upload any required software upgrades to the instrument s Information Technology system with no additional charge to the facility.

PROJECT MANAGEMENT
Vendor will provide delivery, set-up, trash removal, project management, and on-site support during implementation and deployment as necessary/applicable.

USER AND SERVICE MANUALS
The vendor shall provide, at no charge, at least 1 complete and unabridged set of operator manuals, service manuals, troubleshooting guides and parts lists for each piece of equipment purchased for the medical center.

Award shall be made to the lowest priced vendor that adheres to the terms and conditions of the solicitation.

Offerors must complete annual representations and certifications electronically via the System for Award Management (SAM) website located at https://www.sam.gov/portal in accordance with FAR 52.212-3, Offerors Representations and Certifications Commercial Items. If paragraph (j) of the provision is applicable, a written submission is required.

The following contract clauses apply to this acquisition:

All offerors shall submit the following: quotes for the requested items.

All quotations shall be sent to the Daleta.coles@va.gov no later than 10/24/2019 @ 1000.

This is an open-market combined synopsis/solicitation for Idylla Assay Kits as defined herein.B B The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:
"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
OR
"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"
Quoter shall list exception(s) and rationale for the exception(s).

Submission shall be received not later than 10/24/2019 @ 1000 emailed to Daleta.coles@va.gov. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).
