Attached is a Request For Quotation (RFQ), 15B10620QVB170059 for the RADIOPHARMACEUTICAL ISOTOPES at the Federal Correctional Complex Butner, NC. This solicitation closes on SEPTEMBER 16, 2019, at 12:00 P.M., Eastern Time.
This cover letter is intended to provide information regarding the submission of a bid. Please read the enclosed RFQ carefully and complete the Standard Form 1449, Representations and Certifications.
***Failure to complete the Representations and Certifications, can result in non-consideration of your quote.*** All potential quoters are advised that this solicitation includes the clause FAR 52.212-4 , which requires all contractors doing business with the Federal Government to obtain a DUNS number, register in the SAM System (formerly known as CCR), obtain a CAGE Code, identify proper NAICS Codes, properly identify Socio-economic Set-Aside status, SIC Codes, PSC Codes, FSC Codes, obtain an MPIN as well as complete the proper Representations & Certifications (formerly known as ORCA).
If you choose not to respond to this bid, please notify this office by letter briefly outlining your reasons for not responding.
Facsimile Quotes Are Permitted. Submit Only Standard Form 1449, Representations and Certifications.

It is recommended that you ca11 the Contract Specialist prior to mailing your bid to ascertain if there has been a last-minute extension of the aforementioned deadline.
The Contract Specialist assigned cognizance of this procurement is the undersigned. Your attention is directed to the fact that a Contract Specialist is the only individual who can legally commit or obligate the Government to an expenditure of public funds-should a contract result from this RFQ.
Prospective bidders are also cautioned against discussing the preparation of their bid (or any technical questions) with Government technical personnel. The circumstances of such a contact, when verified, may result in non­-consideration of the quotation. Accordingly, all communications prior to an award will be directed to the Contract Specialist, in writing, to the aforementioned address.
Contractor shall furnish an itemized invoice for each delivery at the time of delivery. All deliveries will be counted for verification and signed for by an authorized representative of the government. Contractor must provide a means for electronic payment to include bank routing and account number for all awards over $3,500.00. Payment will be made in accordance with the Prompt Payment Act. You must accept Credit Cards as a method of payment for any award less than $3,500.00.
