Notice: This advertisement is for market research purposes for a potential opportunity. This advertisement is not a solicitation for offers, nor is it a request for proposals. The purpose of this advertisement is to identify potential sources and suitable locations and is not intended to pre-qualify or disqualify any potential offers. The Government will not pay for any costs incurred because of this advertisement. Respondents are advised that the Government assumes no responsibility to award a lease based upon responses to this advertisement

The Altoona VA Medical Center, Altoona, PA desires to identify potential sources that have the capability to provide up to 11,386 Net Usable Square Feet (NUSF) of leased space for a Community Based Outpatient Clinic within the delineated area identified below. Space requirement includes a contiguous space (preferable on one floor but not more than two floors) with ADA (Americans with Disabilities Act) accessibility and VA Barrier-Free Design Standard and Architectural Barriers Act Accessibility Standards, and have 40 parking space, to include 4 ADA spaces or more if required by local law or statute. Space must accommodate PACT (Patient Aligned Care Team) space model (http://www.cfm.va.gov/til/Prototype/CBOCPrototype-02-ProjNarrative.pdf ). The following is an estimate of needed rooms/space:

The anticipated lease will be a full-service lease to mean all utilities, security, telephone, janitorial and all-season landscape and snow removal are included. The anticipated lease term is a firm term of five (5) years, and additional soft term of ten (10) years for a total lease term of 15 years.

The space must be in or abounding the delineated.

Delineated Area: Huntingdon County, PA

West:B Intersection of Routes 305 and 22.B 22 to Bridge Street, to Main Street, to Juniata Valley Pike (305) into the town of Petersburgh, PA.B B Juniata Valley Pike North becomes King Street then turns into Greenwood Road (305), then becomes Shavers Creek Road Northeast (305), Greenwood Road (305/26) Northeast, Greenwood Road (305) Southeast across Belleview. Stay on 305 Southeast to 655/East Main Street.B Take 655 North to Reedsville.B 655 becomes North Main Street in Reedsville, then becomes South Main Street, then becomes Manns Narrows.B

North:B Manns Narrows becomes North Main Street in Yeagertown, turns into South Main street in Yeagertown, then becomes North Logan Boulevard. Turn onto West Freedom Avenue, then becomes East Freedom Avenue, becomes Vira Road.B Turn onto Long Lane Southeast, at State Route 522 in Maitland Southwest on 552 to Crossover Drive into the town of Maitland. From Crossover Drive head Southwest onto Back Maitland Road (RR2004).B Take Back Maitland Road (RR 2004) Southwest to Jacks Creek Lane Southeast and then Southwest.B

East:B Jacks Creek Lane turns into Business Route 22 heading Northwest, to 103/Delaware Avenue South,B 103 Delaware Avenue becomes 103.B Continue Southwest on 103 to the town of Mattawana, PA.B

South:B Continue on 103 Southwest to Croghan Pike in the town of Allenport, PA, which is State Route 522.B Take SR 522 Croghan Pike to Shirley Street through the center of Mount Union, PA.B Take Shirley Street to North Jefferson Street, to State Route 22 West.B Follow State Route 22 West to Oriskany Road West, then North. Oriskany Road is also 655, merges back into State Route 22 West. Stay on State Route 22 West and North back to the intersection of Routes 305 and 22.B Continue on 22 to Bridge Street.




Space will not be within a 100-year flood plain. Space within the 100-year flood plain would only be considered if no space was available outside the 100-year flood plain.

If applicable, historical property should be identified for consideration.

If applicable, socioeconomic classification should be identified for consideration.

Interested parties should submit information sufficient to show space fulfills the basic requirements stated in this notice: outside 100-year flood plain; historical or non-historical space identification; socio-economic status identified, within delineated area; meets the requirement of amount of space and ability to provide PACT design.

Responses to this notice will assist VA in determining whether this acquisition should be set-aside for competition restricted to SDVOSB or VOSB concerns in accordance with 38 USC Sec. 8127 or proceed with Full and Open competition from capable sources.

PLEASE NOTE: Although this notice contains detailed information for interested SDVOSB and VOSB parties, VA encourages all small businesses and other interested open market sources to respond for market research purposes.

SDVOSB and VOSB firms are invited to provide information to contribute to the market research for this project. SDVOSB and VOSB firms must be registered in VA s Vendor Information Pages (VIP) site at https://www.vip.vetbiz.gov/ and have the technical skills and financial capabilities necessary to perform the stated requirements. SDVOSB and VOSB firms are requested to submit a Capabilities Statement to VA for review.

SDVOSB and VOSB firms will provide a Capabilities Statement if they are interested in participating in this project.

Capabilities Statement Will Include:

1. Company name, address, point of contact, phone number, Dunn & Bradstreet number, and e-mail address;

2. Evidence of SDVOSB or VOSB status through registration at VIP (https://www.vip.vetbiz.gov/);

3. Evidence of ability to offer under NAICS Code 531120 and listing in the System for Award Management (www.SAM.gov), including a copy of the representations and certifications made in that system;

4. A summary describing up to three (3) projects of similar size and scope completed in the past five (5) years that demonstrate your company s experience designing, constructing, and managing Federal leased facilities or health care facilities relevant to the project described above. (3-page limit);

5. Evidence of capability to obtain financing for a project of this size.

If desired, the company may also submit a narrative describing its capability.

Although this notice focuses on SDVOSB and VOSB, we encourage all small businesses and other interested parties to respond for market research purposes.

This is not a solicitation for offer, but an inquiry for community based outpatient clinic (CBOC) space.
Expressions of Interest must be submitted to the Contracting Officer electronically to Roberta.Smileybetzler@va.gov by 2:00PM Eastern Time, Thursday, April 4, 2019 and must include the following information:

1. Building name, address and location of available space
2. Amount of offered space in Rentable Square Feet (RSF) and Net Usable Square Feet (NUSF)
3. Building ownership information, brokers or legal representatives must show written
acknowledgement and permission to represent the property
4. Amount of parking available on-site
