Human Factors Program On-Site Support Services
The following changes have been made under amendment 2:
1) Offerors shall request from customers to complete no more than five past performance questionnaires and submit the form directly to the Government utilizing attachment J.5 Past Performance Questionnaires.
2) The Past Performance Questionnaire, Attachment J.5 is attached.
3) Revised RFP is attached.
