Homeward Bound Housing Specialist Services
The State is seeking proposals to provide Homeward Bound Housing Specialist Services. Homeward Bound, Maine’s Money Follows the Person Demonstration, is a program focused on transitioning elderly and/or disabled adults out of institutions and back into their home or community. A Housing Specialist is needed to examine the housing landscape for low income individuals seeking long term care, assist individual program participants in securing affordable and accessible long term housing, and help identify and address system level housing related barriers.
