AMENDMENT ONE (00001) ISSUED TO INCREASE THE QUANTITY REQUIRED FROM SIX (6) TO SIXTEEN (16) AND ADD UPDATED TERMS & CONDITIONS.

Offerors shall complete all required information including but not limited to blocks 8, 14, 15a, 15b, 15c, pricing/delivery information in Section 2, and required/applicable Provision Fill-Ins 52.212-3 Offeror Representations and Certifications--Commercial Items (Oct 2018).

This notice is now a soliciatation - see attachment.   All questions regarding this solicitation must be submitted to the government for clarification no later than 1:00 p.m. on Tuesday, August 13, 2019.

This notice is hereby amended to a Presoliciatation.  The solicitation will be available on or about August 5, 2019 and will be distributed solely through the General Services Administration's Federal Business Opportunities website at www.fbo.gov. All future information concerning this acquisition, including solicitation amendments, will be distributed solely through this website. Interested parties are responsible for monitoring this site to ensure you have the most up-to-date information about this solicitation. Hard copies of the solicitation will not be available. The anticipated date for receipt of quotations is on or about August 20, 2019.  Payments will be made using Electronic Funds Transfer (EFT). All offerors must be actively registered in the Systems for Award Management (SAM) database prior to award, during performance, and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements at www.sam.gov.

Award will be made to the responsible contractor whose offer, conforming to the solicitation, is considered to be most advantageous to the Government based on price and price related factors.

Below is the original Sources Sought FBO Notice.  The Description of the Requirement remains unchanged.

This synopsis is issued in accordance with FAR 5.101(a)(1) and FAR 5.207. This proposed action is for the procurement of Fresenius 2008T hemodialysis systems for the United States Medical Center for Federal Prisoners (USMCFP) located in Springfield, Missouri.

The Government intends to solicit and negotiate with only one source under the authority of FAR 13.106-1(b)(1). This proposed procurement will be a single source award under North American Industry Classification System Code (NAICS) 334510-Electromedical and Electrotherapeutic Apparatus Manufacturing, with a Small Business
Administration size standard of 1250 employees.

Description of Requirement:
2008T hemodialysis system with Clinical Data Exchange -
-Fresenius USA (FUSA) shall supply six (6) 2008T hemodialysis systems to USMCFP Springfield.
-Each 2008T system shall include one (1) Acid/Vinegar Jug.
-FUSA shall also provide one (1) 2008T Operators Manual.
-FUSA shall dispose of existing 2008K or 2008T machines on a one-to-one ratio with the purchase of new 2008T machines within 60 days of new equipment purchase.
-Installation fee for new equipment shall be waived.
-FUSA shall warranty parts and labor for twelve (12) months from the date of installation.
