This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. See attached RFQ.
This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2019-03 Effective 12 Jun 2019 and Veterans Affairs Acquisition Regulation Supplement (VAAR) Updated Part 807, 813, 819 and 853 Effective 25 Jan 2019.
The associated North American Industrial Classification System (NAICS) code for this procurement is 339112, with a small business size standard of 1000.
The VA Northern California Health Care Systems is seeking to purchase Hemosphere Advanced Monitor. Hardware/software purchase must be compatible with hospital information systems, CPRS and PICIS PCs.
Please provide price quotation using the attached RFQ. All price quotes received prior to 06/27/19 will be considered by the Government. Only emailed price quotations received directly from interested persons are acceptable. Receipt must be acknowledged on my desktop computer to be recognized as received. This is the only acceptable method for delivery of quotes.
This solicitation will be set aside for Service-Disabled Veteran-Owned Small Business (SDVOSB) and Veteran-Owned Small Business (VOSB) concerns if there are two or more SDVOSB and VOSB who are capable of providing a quote at a fair and reasonable price. SDVOBs/ VOSBs not listed in are considered non-responsive and price quotations will not be evaluated.
Offerors shall be registered in the System for Award Management (SAM) at the time an offer or quotation is submitted and at the time of contract award. Registration information is located at Award will be made to the responsive/responsible offeror who provides the technically acceptable product at the lowest price.
