COMBINED SYNOPSIS/SOLICITATION:
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation is attached. Solicitation FA460019RA003 incorporates provisions and clauses effective through Federal Acquisition Circular (FAC) 2019-01, dated 20 December 2018. This requirement is an unrestricted (Full and Open Competition) solicitation. North American Industry Classification System Code (NAICS) is 334516 - Analytical Laboratory Instrument Manufacturing, with a size standard of 1,000 employees. All qualified small businesses under this industry are encouraged to submit quotes.

DESCRIPTION OF REQUIREMENT:
Hematology Analyzer Lease and Reagents (includes equipment, supplies/reagents, and training) for Ehrling Bergquist Clinic located at Offutt Air Force Base, NE. The period of performance for this contract is estimated to be 1 March 2019 - 30 Sept 2019 plus four one-year option periods and the six month option to extend services, for a total performance period not to exceed six-one (61) months.

SPECIAL INSTRUCTIONS:
Offerors shall be required to return the attached Standard Form 1449 Solicitation FA460019RA003, signed and filled in appropriately as their offer along with any additional information required to be submitted by FAR 52.212-1 Instructions to Offerors Addendum and any additional fill-in provisions.  Offerors are also required to acknowledge all posted amendments.

The Government anticipates award of a firm-fixed price contract as a result of the solicitation. The solicitation may be downloaded from FedBizOpps, since copies of the solicitation will not be available for mailing. All prospective offerors must be registered in the System for Award Management (SAM) prior to award of contract with any DoD agency. Registration may be accomplished at https://www.sam.gov/.Proposal questions must be submitted in writing (via email) No Later Than Friday 25 January @ 0900 CST.  Please reference Solicitation number "FA460019RA003" in the subject line.  Answers will be posted for all interested parties to review. No telephone questions will be accepted..

LATE OFFERS:
Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in this solicitation by the time specified in the solicitation. Quotations/offers or modification of quotes/offers received at the e-mail address specified for the receipt of offers after the exact time specified for receipt of offers WILL NOT be considered (ref. FAR 52.212-1(f)).
