SOURCES SOUGHT FBO DESCRIPTION
Purpose: The purpose of the Sources Sought for HCHV LDSH services for the Department of Veterans Affairs VISN 15 Kansas City and Leavenworth VA Medical Centers is to collect and analyze information on potential offerors, their capabilities within the market to satisfy the agency s needs, and improve small business access to acquisition information and enhance competition. This is not a solicitation for work, nor is it a proposal.
Background: The goal of the Low Demand Safe Haven (LDSH) Healthcare for Homeless Veterans (HCHV) program is to provide housing and case management services to Veterans and facilitate their access to a broad range of medical, mental health, and rehabilitative services utilizing Safe Haven low demand model guidelines. The target population for this HCHV LDSH is chronically homeless, chronically mentally ill Veterans, Veterans with chronic substance abuse disorders and complex needs. Eligibility will be determined by the VA, based on veteran status; meeting the McKinney-Vento Homeless Assistance Act definition of homelessness (see http://www.hudhre.info/hearth/ for additional information); and an assessment by the VA HCHV program resulting in a determination of serious medical, mental health, substance abuse or other serious psychosocial conditions or stressors making the Veteran highly vulnerable with a high likelihood of death due to homelessness.
Description of Services: The Department of Veteran Affairs, VISN 15 Kansas City Medical Center hereafter referred to as VA KC located at 3801 Linwood Blvd. Kansas City, MO 64128 and Eastern Kansas, Leavenworth Medical Center located at 4101 S 4TH ST., Leavenworth KS 66048 hereafter referred to VA Leavenworth requires Contractors to provide homeless veteran services through a Safe Haven model of residential care at a facilities located within the metropolitan areas of Kansas City MO VAMC and Leavenworth KS VAMC. This will be a multiple award, non-personal Firmed Fixed Price Indefinite Delivery/Indefinite Quantity (IDIQ) contracts. These services, shall not exceed 25 Veterans per day per year for VA KC and not to exceed 10 Veterans per day per year for VA Leavenworth at any one time.
Objective/Milestone: The VA anticipates issuing a solicitation for this requirement during the month of May 2018, dates are estimated only. Solicitation may be posted earlier or later than. Posting will be for a total of thirty (30) calendars days. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments.

NAICS Code is 624221 with a small business size standard of $11M. Based on the Contracting Officers completed Market Research report IAW Far part 10, will depend on how the Solicitation will be issued as full and open competition, or will include Limited Sources. All vendors must be registered in the System for Awards Management database(www.sam.gov) and have input Vet Biz information at http://www.vip.vetbiz.gov . Registration in SAM is free and potential offerors are encouraged to visit the website and register as soon as possible to avoid delays should they be selected for a contract award.
