SOURCES SOUGHT FOR HCHV CERS POPLAR BLUFF VAMC
Purpose: The purpose of the Sources Sought for Healthcare for Homeless Veterans (HCHV) Contracted Emergency Residential Services (CERS) for the Department of Veterans Affairs Poplar Bluff VA Medical Center. It is to collect and analyze information on potential offerors, their capabilities within the market to satisfy the agency s needs, and improve small business access to acquisition information and enhance competition. This is not a solicitation for work, nor is it a proposal.
Background: The Health Care for Homeless Veterans national program is an essential and critical part of Veteran s Health Administration (VHA) throughout the nation. Through the HCHV program, the VA provides outreach/case management services to Veterans and facilitates their access to a broad range of medical, mental, health, and rehabilitative services under 38 U.S.C. 2032, in conjunction with work therapy under 38 U.S.C. 1718 . Emergency housing is a transitional environment that provides a form of supportive atmosphere to Veteran s who suffer from many physiological illness, medical and/or Substance Abuse Disorders (SUD) who currently are living on The Streets , in shelters or experiencing housing instabilities.

Description of Services: The Department of Veterans Affairs, Poplar Bluff Medical Center requires contractors to provide homeless Veteran services through the Healthcare for Homeless Veterans model of residential care at a facility located within the metropolitan area of the John J. Pershing VA Medical Center, 1500 N. Westwood Blvd. Poplar Bluff, MO 63901. This procurement may be a single or multiple award based on housing of male and female within the same facility, non-personal Firmed Fixed Price Indefinite Delivery/Indefinite Quantity (IDIQ) contract(s). These services, shall not exceed a total of 25 Veterans (males and females) Bed days of Care per day per year for Poplar Bluff VAMC. Contract shall be for a one (1) year period of performance, with the intent to exercise four (4) one (1) year option periods.

Place of Performance: This contract will be awarded to a Contractor having an acceptable facility within the Poplar Bluff metropolitan area. If no facilities are available within this area, consideration will be given to the Contractor whose facility meets all the stated requirements and is closest to the Poplar Bluff VA Medical Center.
Objective/Milestone: The VA anticipates issuing a solicitation for this requirement during the month of May 2018, dates are estimated only. Solicitation may be posted earlier or later than. Posting will be for a total of thirty (30) calendars days. The solicitation will be available for download from http://www.fbo.gov. Interested contractors will be responsible for downloading copies of solicitation (and any subsequent amendments) from that website, and no hard copies will be made available. The solicitation will provide full information to contractors about the proposal submission process. Prospective offerors are encouraged to register their interest in the solicitation on the FBO website so that they are notified of the issuance of the solicitation and any amendments.

NAICS Code is 624221 with a small business size standard of $11M. Based on the Contracting Officers completed Market Research report IAW Far part 10, will depend on how the Solicitation will be issued as full and open competition, or will include Limited Sources. All vendors must be registered in the System for Awards Management database(www.sam.gov) and have input Vet Biz information at http://www.vip.vetbiz.gov . Registration in SAM is free and potential offerors are encouraged to visit the website and register as soon as possible to avoid delays should they be selected for a contract award.
