Health Start Services
The Program will have a strong focus on preconception and interconception care through the provision of
educational services, screenings and assessments to women and their families.
The Contractors shall develop administrative, management, organizational systems and information and referral
networks to implement and manage a Program at the contracted site in targeted communities/service
areas at risk for preterm and low birth weight births and high rates of infant mortality per the Health Start Policy
and Procedure Manual (HSPPM). Contractor shall provide monthly program documentation and quarterly reports
with results of efforts related to improving the social determinants of health and achieving any quality
improvement indicators as described in the HSPPM. The program shall be statewide.
