The Program represents a
collaborative effort between the public and private sectors and a cross section
of providers, consumers, and health professionals. In the short term, it seeks to
simplify the manner in which uninsured individuals are screened for financial
eligibility for existing public programs and resources and link them with
appropriate financial and medical resources. In the long term, the Program seeks to
improve individual health status by facilitating the use of primary and preventive
health and reducing the inappropriate utilization of health resources (such as
emergency departments). The selected Health Home/Provider Network vendors
are integral partners to the Department in the effective operation of the Program.
