This RFP is for the provision of medical services to County Department of Health's (DOH) three ambulatory care facilities. In addition, there could also be a need for care provided at community venues. Services shall be provided utilizing a group practice model for three facilities and shall be consistent with acceptable medical standards.
The scope of this proposal presents the necessary qualifications that a contractor for primary care provider services must meet. All work and services will be performed during the hours agreed upon by DOH and the contractor. The contractor should submit a proposal that includes provision of primary care provider services.
