HYDRAULIC LOADING DOCK LIFTS (PIT MOUNTED)
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Part 13.105, Department of Veterans Affairs, VAMC, Syracuse, 800 Irving Avenue, Syracuse, NY 13210, intends to award a firm, fixed price contract to provide Hydraulic Dock Lifts, removal of the old lifts and installation of the new lift. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA528-14-Q-0304 is issued as a request for quotation. This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This RFQ is being issued as a Small Business Set-aside. Submission of proposals must be in accordance with FAR 52.212-1, Instructions to Offerors, Commercial Items.
The Government will award a firm, fixed-price contract to the responsible Offeror whose offer conforming to the solicitation provides the Best Value to the Government, price and other factors considered; capability of the item meeting the attached specifications.
The applicable NAICS code is 423830 and the size standard is 100 Employees. This is 100% set aside for Small Business. The VA will not pay any costs incurred in preparation and submission of proposals.
The Contractor shall deliver FOB Destination to the Department of Veterans Affairs, VAMC, Syracuse, 800 Irving Avenue, Syracuse, NY 13210. All responsible Vendors shall provide an offer for the following "Brand Name or Equal" Products:

Offerors shall include a completed copy of the provision FAR 52.212-3 Offeror Representations and Certifications-Commercial Items which is completed by registering in the System for Award Management (SAM) Website at: https://www.sam.gov/portal/public/SAM/. Proposals must also include contractor's DUNS Number, prompt payment terms, and correct remittance address. OFFERORS MUST BE REGISTERED WITH SAM PRIOR TO CONTRACT AWARD.
Offerors are required to submit descriptive literature with their quote.
The date for receipt of proposals is May 30, 2014 at 2:00 PM EDT. All technical and price proposals must be submitted electronically (via email) to [email protected]. Written questions must be submitted before 12:00 p.m. Eastern Daylight Time on May 27, 2014. All responsible offerors may submit a quote which shall be considered by the VA Medical Center.
