(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.
(ii) The solicitation number for this effort is 12639518Q0022. This solicitation is issued as a Request for Quote (RFQ). SEE ATTACHED.
(iii) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-96, effective November 6, 2017.
(iv) This procurement is issued as a total small business set-aside.
(v) Contract line item number(s) and items, quantities and units of measure: See attached.
(vi) Description of requirements: Work involves performing preventative maintenance services as well as a routine and emergency repairs. See attached.
(vii) Date(s) and place(s) of delivery and acceptance at FOB point: See attached.

Quoters are advised to carefully review the attached instructions for submitting a quote.
A COMPLETE QUOTE SHALL INCLUDE THE FOLLOWING INFORMATION:
1. Cover page or signed standard form SF1449 and acknowledgement of Amendments, if any, to the solicitation.
2. Completed Pricing Schedule (Pgs. 6 - 7). Alternate Pricing Schedules will not be considered. The Government will award on an "All or None" basis. Quotations must include a price for all items to be eligible for award.
3. Submit Attachment 1, Project Experience Questionnaire (Pgs. 4 - 5) for a minimum of three (3) projects to demonstrate relevant experience performed within the past five years. Relevant experience is defined as preventative maintenance service and repair services for the same or similar HVAC equipment.
4. A letter of commitment is required in order for a proposed subcontractor's Project Experience to be evaluated.
