 HVAC Inspection and Diagnostic Report
1.  This solicitation is for a commercial supply prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; this is a Request for Quotation (RFQ) and a written solicitation will not be issued. The solicitation number is FA4661-18-Q-0106.


2.  The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-99 effective 15 Jun 2018, DFARS DPN 20180601 effective 30 May 2018, and AFFARS AFAC 2018-0525 effective 25 May 2018.


3.  The North American Industry Classification System code (NAICS) is 238220, Plumbing, Heating, and Air-Conditioning Contractors , and the Product Service Code (PSC) is J045, Maint/Repair/Rebuild of Equipment- Plumbing, Heating, and Waste Disposal Equipment.


4.  Set-aside:  100% Total Small Business


5.   Place of Performance:

6.  Period of Performance:  10 Days after contract award

7.  All questions pertaining to this requirement must be received no later than 27 Jul 2018, 2:00 P.M. (CST).              


8.  Deadline for receipt of offers: 03 Aug 2018, 2:00 P.M. (CST). 


9.  QUOTE SUBMISSION: All quotes must be emailed to SrA Lydia Simpkins via email at lydia.simpkins@us.af.mil or MSgt Brock Bell at brock.bell@us.af.mil. ; For any questions call 325-696-1456.  If you e-mail your quote, page 2 (or equivalent*) must be filled out.  Your offer shall include, but is not limited to the following:

•1.  Complete (must be signed) Page 2 of the Solicitation (see attachments for full solicitation) (or contractor equivalent*)

•2.  Copy of Environmental Protection Agency (EPA)/ Chlorofluorocarbon (CFC) Universal Certification. 

* The offeror is not required to use Page 2 of this Solicitation; however, if you chose not to use it, all of the information on Page 2 must be included in your offer.

10.  Evaluation of Offers: The evaluation for this acquisition will be solely based on lowest price of items that are found to be technically acceptable.  The offeror's proposed price will be evaluated for completeness, and reasonableness by signing its offer, the offeror certifies that each price stated on each CLIN includes an appropriate apportionment of all costs, direct and indirect, all state and local taxes, overhead, and profit.
