We are in need of an expert PHP programmer. Successful candidate will know how to create classes, connect to a preexisting database, and insert sanitized data submitted by HTML form. The existing table holds about 25 fields with additional TIMESTAMP and auto-increment fields. The HTML is a 3-step process involving: (Also see attached PDF)

STEP 1: Inserting the first few fields which involves the users contact information and desired services;

STEP 2: Details on each service chosen in step one (there are only 3 services to choose from) will generate a new mini HTML form on page 2. So page 2 will have up to 3 forms. Each form will write data to a different table and notify the superadmin via email. The mini forms will each insert approximately 13 fields (pages 4 and 5 of PDF).

Also based on which services are chosen, there will be text inserted into the "Terms of Service" area. The terms will already have default text on the page, with additional paragraphs related to whatever service chosen. The user will have to "sign" the webpage by placing his/her full name in a textbox and clicking an "I AGREE" button. The name must match a field previously listed in Step #1 (namely namePOC on page 1) The text from this area may be inserted into a BLOB field (legalTOS on page 3).

STEP 3: This is a confirmation screen that sends an email to the super administrator, a welcome email to the new member (with a randomly generated username and password), and displays instructions on where to click to login.

There are other PHP related tasks if you are able to complete this to specification. This must be done extremely quickly and without error. (That includes spelling errors.) We expect the use of classes and server side form validation using PHP. You will be required to label each form field as noted in the attached PDF and use the existing database. You may also be required to use existing classes (unless you wish to rewrite them) to process form data.
