HIV/AIDS MEDICAL CASE MANAGEMENT SERVICES

Medical case management services are a range of client-centered services that link clients with
health care, psychosocial, and other services. The services ensure timely and coordinated access to
medically-appropriate levels of health and support services and continuity of care, through an
ongoing assessment of the clients needs and personal support systems and the coordination and
follow-up of medical treatments. Medical case management includes the provision of treatment
adherence counseling to ensure readiness for, and adherence to, complex HIV/AIDS treatments.
The contractors primary activity will be to provide medical case management services, including
treatment adherence, to people living with HIV/AIDS.
All programs funded under this RFP must incorporate the following into their proposed HIV
Medical Case Management programs:
1) Assisting clients in adherence to HIV drug regimens/HIV therapies
2) Linkage and referral to:
HIV care and treatment services
HIV prevention services
Partner Counseling and Referral Services
Sexually Transmitted Disease screening and treatment
Viral Hepatitis screening and treatment
