GYM/FITNESS MEMBERSHIPS FOR THE US COAST GUARD


IF YOU WISH TO QUOTE THIS REQUIREMENT PLEASE HAVE QUOTES TO ME NO LATER THAT 4:00 PM, MONDAY, 23 JANUARY 2017


REQUIREMENTS


1. Provide 180 gym/fitness memberships to all vendor locations in the Topeka and surrounding area.
2. Provide list of gyms and their locations
3. Provide a list of pertinent amenities included with the gym membership. ie. Fitness classes, etc.
4. Companies quoting should include the following in their quote:
(a) the solicitation RFQ number.
(b) The name, address, telephone number, and point of contact of the offeror.
(c) Price the base year and all subsequent options years to arrive at a 5 year total cost. We are a Tax Exempt Agency; please do not include Federal or State Taxes in the quoted price.
(d) Company Tax Information Number and DUNS Number. Vendor must be registered or agree to register with System for Award Management, www.sam.gov , formerly known as Central Contractor Registration, www.ccr,gov before the award can be made.
(e) Completed copy of your company's Representations and Certifications per FAR 52.212-3 Offeror Representations and Certifications - Commercial Items. This can be completed electronically at https://www.sam.gov/portal/public/SAM/. Offerors must submit a complete copy with their offers.
(f) All responsible sources may submit a quotation, which if timely received, shall be considered by this agency.


BASE YEAR WITH (4) OPTION YEARS


This contract will be a base year with (4) options years for a total of 5 years. Each subsequent year following the base year will be exercised by mutual agreement between the vendor and the US Coast Guard. Each option year will be "Subject to the Availability of Funds" IAW FAR Part 52.232-18.

52.217-6 -- Option for Increased Quantity.
As prescribed in 17.208(d), insert a clause substantially the same as the following:
Option for Increased Quantity (Mar 1989)
The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within [insert in the clause the period of time in which the Contracting Officer has to exercise the option]. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree.

52.217-7 -- Option for Increased Quantity -- Separately Priced Line Item.
As prescribed in 17.208(e), insert a clause substantially the same as the following:

The Government may require the delivery of the numbered line item, identified in the Schedule as an option item, in the quantity and at the price stated in the Schedule. The Contracting Officer may exercise the option by written notice to the Contractor within [insert in the clause the period of time in which the Contracting Officer has to exercise the option]. Delivery of added items shall continue at the same rate that like items are called for under the contract, unless the parties otherwise agree.

52.217-8 -- Option to Extend Services.
As prescribed in 17.208(f), insert a clause substantially the same as the following:
Option to Extend Services (Nov 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within ______ [insert the period of time within which the Contracting Officer may exercise the option].

52.217-9 -- Option to Extend the Term of the Contract.
As prescribed in 17.208(g), insert a clause substantially the same as the following:
Option to Extend the Term of the Contract (Mar 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within _____ [insert the period of time within which the Contracting Officer may exercise the option]; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least ___ days [60 days unless a different number of days is inserted] before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed ___________ (months)(years).

52.232-19 -- Availability of Funds for the Next Fiscal Year.
As prescribed in 32.706-1(b), insert the following clause in solicitations and contracts if a one-year indefinite-quantity or requirements contract for services is contemplated and the contract
(a) is funded by annual appropriations and
(b) is to extend beyond the initial fiscal year (see 32.703-2(b)):
Availability of Funds for the Next Fiscal Year (Apr 1984)
Funds are not presently available for performance under this contract beyond ________. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond _____, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer.
