THIS IS A SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS

The Department of Veterans Affairs is conducting a MARKET RESEARCH SURVEY to obtain information regarding: (1) the availability and capability of qualified large business sources (2) the availability and capability of qualified small business sources; (3) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concern, (4) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition, and (6) current market price. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible.

DESCRIPTION OF THE REQUIREMENT:

The Albany VAMC, 113 Holland Ave, Albany, NY 12208 is seeking a service maintenance agreement for the GE imaging equipment listed below.

Discovery PET/CT690 (VCT 64-slice). This coverage shall include full support for all components and software, including an associated Advantage Workstation 4.6, Pin Source Coverage, and System and Tube Coverage.
3 Optima XR220AMX Mobile X-ray Systems. This coverage shall include full support for all components and software, including three (3) Flashpad Digital Wireless Detectors, Tube Coverage, Auto Exposure Control, and Battery Replacement Coverage.
Lightspeed VCT 64 slice. This coverage shall include full support for all components and software, including an associated Advantage Workstation 4.4, Pin Source Coverage, and System and Tube Coverage.
2 Discovery XR656 Systems. This coverage shall include full support for all components and software, including three (3) Flashpad Digital Wireless Detectors, Tube Coverage, Auto Exposure Control, and Battery Replacement Coverage.
Discovery CT750 HD. This coverage shall include full support for all components and software, including an associated Advantage Workstation 4.7, Pin Source Coverage, and System and Tube Coverage.

The service maintenance agreement shall include the following, all work shall be done by authorized Philips technicians and use only new Original Equipment Manufacturer (OEM) parts.
Priority Remote Service response (requires connection to Philips Remote Services network).
All labor and travel during standard hours of coverage - Mon-Fri 8am - 5pm, excluding holidays.
One hour initial response with four hour on-site response.
All spare parts that fail during normal use. (Excluding consumables)
Parts Delivery 10:30AM Next Day.
Equipment Planned Maintenance services per Philips manufacturer specifications performed during standard hours of coverage.
Proprietary operating system software enhancements without hardware changes (excludes software upgrades).
Unlimited Technical Services (24x7) & Clinical Applications Telephone Support (Mon-Fri 8am - 5pm)
Remote Service Support (includes Remote Desktop).
Remote Proactive Monitoring (requires connection to Philips Remote Services network).
Reports: PSI Eligible, service history, PM's on request.


All responsible sources may submit a capability statement, proposal, or quotation which shall be considered by the Albany VAMC. This is not a solicitation.
