This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.

This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.

This is a Firm Fixed Price Request for Quotation (RFQ) being issued by the Nevada Army National Guard (NVARNG) United States Property and Fiscal Office Purchasing and Contracting Division for Aviation Medical Readiness in Support of Individual Medical Readiness (IMR) and Fully Medically Ready (FMR) at specified dates between August 6, 2019 and April 2, 2020 at NVARNG locations in Reno and Carson City, Nevada.

This solicitation is prepared in accordance with FAR Part 12, the acquisition of commercial items in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed in FAR 13, Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect
through Federal Acquisition Circular (FAC) 2019-03.

This solicitation is a TOTAL SMALL BUSINESS SET-ASIDE and the North American Industry Classification System (NAICS) code for this solicitation is 621111 with an industry size standard of $11 million and the Product Service Code (PSC) is Q403.

A responsible offeror/quoter is one whose offer conforms to the solicitation and will be most advantageous to the Government. 

All firms or individuals responding must meet all standards required to conduct business with the Government,including registration with the System for Award Management (SAM), and be in good standing. All qualified responses will be considered by the Government. In addition to providing pricing each quote must include
vendor's CAGE/DUNS number, and must include point of contact information. Failure to provide SAM registration may result in your quote being disqualified from consideration. IAW FAR 52.204-7 and DFARS 252.204-7004 Alt A, prospective vendors must be registered in the SAM database prior to award. Prospective vendors should visit the SAM website at https://beta.sam.gov/ to register.

Quoted pricing can be submitted via company formatting, or the SF1449 can be completed and submitted as the price quote. The SF1449 provides the contract clauses, terms and conditions for this requirement.

Quotes and any related documentation shall be submitted to Greg Kothman at gregory.p.kothman.civ@mail.mil prior to 11:59 PM Pacific Time on Monday, July 22, 2019. This is the only means of submitting quotes in response to this RFQ.
