This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) The Navajo Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Proposal (RFP) 19-245-SOL-00011. This solicitation is issued on a SERVICE DISABLED VETERAN OWNED SMALL BUSINESS SET-ASIDE.
(iii) The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16.
(iv) The associated North American Industry Classification System code is 333913 and the small business size standard is 750 employees.
(v) PRICE/COST SCHEDULE - Contractor shall include all costs on fee schedule to complete services.
(vi) PURPOSE OF THE CONTRACT: Task Description: Contractor shall provide all labor, material, equipment, etc. for a removal and replacement of new fire pump at the Four Corners Regional Health Center. See attached Soliciation.
(vii) Date and Place of Delivery: Four Corners Regional Health Center, Jct. Hwy 160 & Navajo Route 35, Red Mesa, AZ 86514
(viii) Specifications: See attached Solicitation.
(ix) Evaluation Factors: The Government reserves the right to issue a single award or multiple awards to the schedule contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following evaluation factors will be considered:
1. Past performance; See section L of Solicitation for Subfactors
2. Technical Capability; See section L of Solicitation for Subfactors
3. Cost
These evaluation factors will be weighted more than price. Price will be utilized as a tiebreaker in case of equally acceptable proposals.
CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer.
AWARD DECISION: The Government will award a contract, resulting from this solicitation, to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
PROPOSAL SUBMISSION INSTRUCTIONS:.All responsible offerors may submit a proposal, which shall be considered via email to Kerri.Gilmore@ihs.gov, Department of Acquisition Management and Contracts, no later than 3:00 p.m., MDT on April 1, 2019.
