Family/Professional Partnership
The Connecticut Department of Public Health (DPH) is pleased to announce the availability of funds to provide the following activities:
Statewide outreach and culturally-competent education to families/caregivers on the concept of medical home and a family-centered approach to care for Children and Youth with Special Health Care Needs (CYSHCN);
Develop and expand family/professional partnerships; and,
Link or identify and provide support in linking children and adolescents with special health care needs to medical homes and other family support services.
