Extended Acute Care (Community Based and/or Hospital Based)
The County is interested in receiving proposals that describe service delivery systems and services that can be provided for persons as diversions from placement at Allentown State Hospital (ASH); or as a community resource for discharge from a community hospital, or ASH. The County is seeking a range of 12-20 beds for the region. Proposals submitted for Non-Hospital Based EAC cannot exceed a total bed capacity of 16.
