Evaluation Plan Consultant
Product/Service Description: The Vermont Department of Health Fit and Healthy Vermonters program is seeking a Contractor to design and write a program evaluation plan. This scope of work includes the design and development of the evaluation plan. Note: it will not include gathering new data or conducting data analysis. The evaluation plan will be for implementation of the Fit and Healthy Vermonters plan for obesity prevention including short term, long term and process outcomes. The obesity prevention plan can be found on the Vermont Department of Health's website at http://healthvermont.gov/family/fit/target.aspx. The written evaluation plan must be developed in partnership with staff from Vermont's Fit and Healthy Vermonters obesity prevention program and Division of Health Surveillance. The evaluation plan will follow the Centers for Disease Control and Prevention Framework for program evaluation. Duties include: 1. Weekly meetings with Fit and Healthy Vermonters Program staff and Health Surveillance Staff for coordination of the development of the evaluation plan. 2. Identifying appropriate data sources, data collection methods, and instruments for assessing Fit and Healthy Vermonter program activities. 3. Reviewing available data sources and methods to create a comprehensive evaluation plan for the Fit and Healthy Vermonters program. 4. Producing a written evaluation plan to be completed prior to December 31, 2007.
