REQUEST FOR PROPOSAL:Evaluation of the First Pilot Phase of the Ontario Childhood Healthy Weights Surveillance System
Purpose: To develop and implement an evaluation plan, using the draft plan in Appendix A as a guide, in order to produce an evaluation report of the first pilot phase of the Ontario Childhood Healthy Weights Surveillance System. The evaluation report will guide any future planning and implementation of the surveillance system.
