Establish and Operate Health-Related Services At Senior Multi-Service Centers
The City of San Antonio is seeking proposals from Respondents qualified to Establish and Operate Health-Related Services on a daily basis. These services will be offered by the Respondent AT NO COST to the City for seniors, age 60 and older, in senior one-stop multi-service centers, located in City Council Districts 4, 5 and 10. The City reserves the option to add additional sites, also as specified by the City. Services will include core health services, as well as proactively addressing chronic conditions and providing acute care to clients. The health-related services will enhance the comprehensive social service delivery system already offered at the multi-service centers. A Pre-Submittal Conference will be held at the Carver Center, 226 Hackberry, San Antonio, Texas 78202, at 1:00 p.m., Central Time, on February 6, 2009. Respondents are encouraged to prepare and submit their questions in writing five (5) calendar days in advance of the Pre-Submittal Conference in order to expedite the proceedings. Attendance at the Pre-Submittal Conference is optional, however highly encouraged.
