Employee Assistance Program
The City of Peoria, Arizona is soliciting proposals from qualified firms to provide an employee assistance
program for the City of Peoria employees and their dependents. The services include but are not limited to:
Short-term out-patient counseling (a minimum of 12 sessions each per year for all employees and their
dependents with reduced costs for additional recommended sessions)
24 hour crisis intervention
Information and Referral
Psychological & Educational Evaluation/Testing
Psychiatric Services (okay if referral)
Alcohol & Drug Treatment (okay if referral)
Legal & Financial Problems
Child and Elder Care
Self-Help Groups
Supervisory Training for managers / supervisors on the EAP program and on managing the troubled
employee, and drug abuse information (ADOT and USTA compliance).
Supervisory / management consultation for managing troubled employees (guidance on City policies and
procedures for documenting, confronting, referring and monitoring job performance).
EAP promotion through brochures and attendance at the Citys Benefits Open Enrollment Fair.
Annual and quarterly utilization reports.
Provide knowledgeable staff to present on various topics at Wellness events when requested by the City.
