This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

The solicitation number for this acquisition is 020-G48-18-0112. The solicitation is issued as a request for quotation (RFQ) using FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures.

The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97, effective January 24, 2018.

The North American Industry Classification System (NAICS) Code for this acquisition is 621111 - Offices of Physicians (except Mental Health Specialists) with a business size standard of $11.0 Million.

The Navajo Area Indian Health Services, Gallup Indian Medical Center (GIMC), Purchasing Department has a requirement for Emergency Medicine Physician Services to be performed at the Gallup Indian Medical Center, Gallup, New Mexico. The need calls for Physicians to be board certified or board eligible in Emergency Medicine. All services are to be performed in accordance with the attached Performance Work Statement (PWS) No. GIMC-18-001.

The proposed required services are to commence as specified in the FY 18 Pricing Schedule.

Period of Performance: date of award to December 31, 2018.

Place of Performance: Gallup Indian Medical Center, 516 E. Nizhoni Blvd, Gallup, NM 87301.

The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition.

FAR 52.212-2, Evaluation-Commercial Items, The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote(s) represents the best value as defined by FAR 2.101 and results in the lowest-cost alternative for the Government. In determining best value, the following factors will be considered: Price, Past Performance, and the Qualifications of each proposed candidate. As part of the evaluation process, candidates may be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform the required services. The socio-economic status of an offeror may also be considered should more than one quote represent the best value and lowest-cost alternative

Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer.

FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Jan 2017) applies to this acquisition.

FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items (Jan 2018), applies to this acquisition.

-Complete candidate profile (incomplete profiles will not be considered for award)
-Certification Statement for each proposed candidate. (All elements requiring a date are to be complete and accurate.)

Interested parties are requested to submit a quote, along with all completed supporting documents as soon as possible.

**Due to the immediate need for Physician Services, we will begin to review only completed documents (Quote, Pricing Schedule, Certification, and Profile). Awards may be established before the closing of this synopsis/solicitation.
