This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation or commitment by the Government. This notice is intended strictly for market research. The purpose of this Sources Sought notice is to obtain information from qualified vendors with special interest relative to the North American Industry Classification Code (NAICS) 339112, Surgical and Medical Instrument Manufacturing.

The Government will not pay for information received in response to this Sources Sought, nor will the Government compensate any respondent for any cost incurred in developing the information provided to the Government. This Sources Sought does not constitute a commitment by the Government. Information provided in response to this market survey will be used for informational purposes only and will not be released. Vendor participation is not a promise of future business with the Government.

Requirement
The VA Ann Arbor Healthcare System has a brand name only requirement for (1) Ellex Tango SLT/YAG Combination Laser Console, (1) Total Access Wheelchair Accessible Mobile Table (V-Base), (1) Ocular Latina SLT Gonio leans with flange, (1) Ellex Laser Service Manual, (1) Beam Splitter, (1) Observation Tube, and (1) Extended Warranty. Typically, YAG lasers and SLT lasers have been manufactured as separate stand-alone laser systems. Ellex Tango features a proprietary dual-mode laser cavity. The use of the YAG Laser combined with the ability to perform SLT (Selective Trabeculoplasty) will allow Ann Arbor VAMC to offer a service currently being sent out to the community. The Tango provides SLT and YAG capabilities in a single, advanced laser system. On-site delivery shall include installation of equipment and 8-hour-in-service training for all staff by an OEM certified installer. Delivery shall be made to the VA Ann Arbor Healthcare System, 2215 Fuller Road, Ann Arbor, MI, 48105.
Interested vendors are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract. Registration is available at www.sam.gov.

The deadline to respond is July 17, 2019 at 4:00 pm (E.S.T).
