The Department of Public Health seeks information regarding adaptable and available commercial software to implement an internet-based electronic death registration system (EDRS) in the state of Connecticut. The EDRS must facilitate the immediate reporting, collecting and processing of death data by funeral directors, physicians and medical examiners throughout Connecticut; provide for registering and issuing certificates of death; allow for issuance of burial transit permits and cremation approvals; and analyze cause of death data. In addition, the system must provide real-time verification of social security numbers online, for reporting the fact of death to SSA, and ad-hoc report writing. Though the focus of this project is to develop the death registration component, the system should also have the potential to be expanded to a fully integrated vital records system (EVR), with complete functionality for all vital records including but not limited to birth, death, fetal death, adoption, marriage and civil unions. The selection process for a vendor will be a two step process. The first step will be to gather preliminary information on qualified organizations through responses to this RFI. The second step will be to obtain detailed proposals, including fee arrangements, in response to an RFP.
