Subject: Solicitation VA247-15-Q-0225, Purchase, Deliver, and Organize Ebola Cache PPE Kits - Request for Quote
Dear Prospective Offerors:
The VA Medical Center invites your firm to submit a quote for the subject requirement as indicated in the Statement of Work and Price/Cost Schedule of the solicitation.
The Government intends to make an award of a firm-fixed price contract for this procurement.
Please submit a quote in accordance with the following instructions:
1. Complete blocks 12, 17a, 30(a) (b) and (c) of the attached SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the Solicitation. The Solicitation constitutes the model purchase contract.
2. Complete Contractor information on page 3 of the solicitation
3. Complete PRICE/COST SCHEDULE starting on page 9 of the solicitation
4. Complete section E.4 (a) on page 33 of the solicitation
5. Read the 1st paragraph of section E.2 on page 20 of the solicitation. Complete paragraph (b)(2) on page 23 by writing "NONE" in the blank if if you completed the online reps and certs.
6. Submit any quotes on company letterhead also
Your reply to this request for quote must set forth full, accurate, and complete information as required by this solicitation. Your quote shall be signed by an official authorized to bind the firm and be submitted via email on or before 8:00 a.m. CST on Tuesday, February 3, 2015 .
