EMPLOYEE ASSISTANCE PROGRAM
THE CITY and its Agencies & Authorities is requesting proposal for a professional consultant to provide the following Employee Assistance Program (EAP) professional services:
Upon the request and need of an eligible employee of the City desiring assistance, assessment/brief therapy/referral will be given during the term of this agreement for each eligible employee of THE CITY AND ITS

AGENCIES & AUTHORITIES and such of that employee’s spouse and dependents.
The purpose of these sessions will be for assessment of problem and referral if necessary. If it is determined that an employee or family member needs to be referred for further or more specialized services, the CONTRACTOR will provide such a referral and follow up. In such cases, the referred person(s) will be financially responsible for those services they receive either through health insurance benefits or self-pay. The definition of “eligible employee” refers to any employee who is in active service with THE CITY AND ITS AGENCIES & AUTHORITIES, his/her spouse and dependents.
